RMV Documentation Request
Please use this form to request your transcript and letter of enrollment to be used for your Massachusetts' RMV Learner's Permit/License.  The RMV requires 2 documents that show your full name, date of birth and address in order to apply for a Real ID Permit/License. These requested documents fulfill the RMV requirement.  

Completed packets can be picked in the School Counseling Department at the secretaries' desks.
